These two forms above above are my Hazard Evaluation forms for my photography location Recce. The first one is where I fill in the details for the photo shoot and then circle the hazards that could occur when I am taking photos for my magazine. I circled these hazards, which could occur when i am taking photos and they are, cables, costume/make up, electricity, lighting, slipping/ tripping/ falling and the weather, as my main hazards.
The second sheet was where I evaluate three of the hazards that I had circled in detail.
